I do not think a woman lactates on nipple stimulation or sucking alone. When a woman has a baby, the body releases a horomone that allows the milk to come in. I have been told its when the placenta seperates from the body.

They can induce lactation with meds and hormones. A woman I know was adopting a child and she was going to breastfeed. Still it is not natural and sort of freaks me out a little. It was wierd to see her with these huge boobs and nipples that stuck out when she was never prego.

I am nursing my little one. My milk came in when he was 3 days old. The female body is an amazing thing.

Two ways: mechanical stimulation (i.e. nipple massage, baby suckling, breast pump)
OR hormones (a big dose of estrogen which you stop suddenly, followed by some prolactin drugs).
Either way, the milk may not come in fully and you have low quality colostrum. Most people just do it for the bonding experience and supplement with formula. The wife in this case has two kids from before, so it is possible to get some milk going.

Men in his situation may commonly not have genital surgery but they also don't pick and choose when they want to be a man and when they want the biological functions of a woman. That is why 75% of female to male patients (hundrends of thousands of americans) only get mastectomies and hormone therapy and they are LEGALLY male.
